HOW TO MAKE AN APPOINTMENT

Booking an Appointment
Most exams and procedures, including CT scans and ultrasounds, require an appointment, which can be scheduled over the phone with any of our clinics. In general, plain X-Ray exams, such as chest X-Rays, do not need an appointment; however, certain specialised scans may require one. If you are unsure, please contact one of our clinics. We understand that some patients may feel nervous before their procedure, so we aim to arrange appointments as promptly as possible.

Pre-examination instructions will be provided by our reception team when you book your appointment.

What to Bring to Your Appointment:

  • Request/Referral from your doctor or referring provider: This is a medicolegal requirement necessary to ensure that the Medicare rebate can be processed.
  • Previous reports or images: Any past reports or images related to the scan or procedure you are having.
  • Medicare card: Required for processing your Medicare rebate.
  • Concession cards: Health Care and Pension cards or DVA (Department of Veterans’ Affairs) cards.
  • WorkCover details: If applicable, please bring the details of your employer’s insurance company and your claim number.

Please note that Clearview Medical Imaging accepts all referral forms.

ARRIVAL

Please arrive at the clinic 15 minutes before your scheduled appointment to complete the necessary forms and registration process. Be sure to bring your referral and any relevant previous imaging or reports.

PAYMENT/BILLING POLICY

Clearview Medical Imaging offers bulk billing for most Medicare-eligible services. Please speak with our reception staff for further details.
